I represent that it is my desire and intent that the child identified below participate in the activities at The Book Kitchen LLC, specifically including cooking classes. I also represent that I have the authority to enter into this agreement on behalf of my child as the child’s parent or legal guardian. I acknowledge, agree, and understand that my child’s participation is voluntary, and I assume the potential risks associated with the activity in which he or she take part of. 

On behalf of my child, I expressly and voluntarily assume all risks associated with his or her participation in a Book Kitchen LLC class. I understand that the activity that my child is participating in may be of a hazardous nature, accidents occur and that participants can sustain personal injuries. Knowing the risks involved, I agree to assume all risks of injury to my child. I hereby release, discharge and hold harmless The Book Kitchen LLC, its officers, directors, members and managers, and all other members of its cooking school staff whether contracted or employed, from and against all liability, damages, personal injury, or property damage which my child may have, or which may accrue, arising out of or connected to in any way with my child’s participation. I further understand and agree that this waiver, release, and assumption of risks is to be binding on my child’s heirs and assigns.

Food Allergies and Sensitivities. Please contact us at least one week in advance if you have food allergies or sensitivities. You are responsible for advising us of the type and severity of your condition. We will try to accommodate you to the best of our ability, but please understand that menus are prepared in advance, and last-minute changes are generally not accommodated. Reactions due to food allergies and sensitivities are included as risks associated with culinary classes.

I agree that this Agreement shall be governed and interpreted under Wisconsin law, and I acknowledge that I have read and understand this document, and by signing this form, I agree to the above hold harmless, waiver and release.
